This document is a required part of your clinical file under Mexican Official Norm NOM-004-SSA3-2012 and must be read in full before signing. By signing, you declare that you have understood the nature, benefits, risks, and limitations of the service you will receive.
1. Nature of the service
Regeneris Therapy delivers telemedicine services through secure videoconference platforms (Daily.co, Cal.com) with duly licensed physicians whose professional IDs are registered with Mexico's SEP.
A videoconsultation is a remote medical act. It allows clinical assessment, guidance, and prescription, but it does not replace in-person physical examination nor emergency care. In case of an emergency you must immediately go to the nearest hospital emergency room or call 911.
2. Expected benefits
Expected benefits of the service include, among others:
- Access to specialized medical assessment without needing to travel.
- Continuity of follow-up for prescribed peptide treatments.
- Guidance on dosage, administration, expected effects, and warning signs.
- Electronic prescription valid under the applicable regulatory framework.
- Privacy and comfort while receiving care from home.
3. Risks and limitations of telemedicine
Telemedicine has limitations inherent to the remote format which the patient acknowledges and accepts:
- Inability to perform direct physical examination, auscultation, or in-person vital-sign measurement.
- Dependence on internet, audio, and video quality; technical failures may interrupt the consultation.
- Small risk of outages or malfunctions on the technology platforms used.
- The physician may conclude, after the videoconsultation, that in-person evaluation or additional tests are required before issuing a prescription.
- The information you provide (symptoms, history, medications) is the basis for clinical judgment; incomplete or inaccurate data can affect the quality of the diagnosis.
4. Peptide treatments: side effects and contraindications
Bioactive peptides are molecules prescribed under medical judgment. Although they are generally well tolerated, known side effects may occur which the patient declares to understand:
- Local reactions at the injection site: mild, transient redness, pain, swelling, or itching.
- Less frequent systemic reactions: headache, nausea, fatigue, dizziness, appetite changes, fluid retention, or transient glucose alterations.
- Allergic or hypersensitivity reactions, including very rarely anaphylaxis.
- Potential interactions with other medications, supplements, or preexisting medical conditions.
- General contraindications: pregnancy, breastfeeding, active cancer or recent oncologic history, decompensated autoimmune disease, significant hepatic or renal insufficiency, and known allergy to the active ingredient or excipients. The physician will assess each case.
- Results vary between individuals and are not guaranteed; response depends on adherence, lifestyle, and individual biology.
5. Confidentiality and data protection (LFPDPPP)
Your personal data, including sensitive health data, are processed by Regeneris Therapy as controller under Mexico's Federal Law on Protection of Personal Data Held by Private Parties (LFPDPPP), its Regulations, and INAI guidelines.
The videoconsultation is not recorded unless you give express consent for clinical or improvement purposes. Clinical records are retained for the period required by NOM-004-SSA3 and protected by reasonable administrative, technical, and physical safeguards.
Data may be shared only with strictly necessary medical and administrative staff, with technology providers acting as data processors (Cal.com, Daily.co, Supabase, Stripe, Resend, Cloudflare), and with health authorities when legally required.
6. Patient rights (ARCO)
As data subject you may exercise at any time your rights of Access, Rectification, Cancellation, and Objection (ARCO), and revoke your consent, by written request to privacidad@regeneristherapy.com.
Additionally, as a patient you have the right to: receive dignified and respectful treatment; know the diagnosis, treatment, and prognosis; freely decide about your care; grant or refuse consent; be treated confidentially; seek a second opinion; be cared for by licensed staff; and file complaints with CONAMED or the competent authority.
7. Patient responsibility
By signing this consent you commit to:
- Provide truthful, complete, and up-to-date information about your medical history, medications, allergies, and relevant conditions.
- Follow medical instructions on dosage, route, and frequency of administration precisely.
- Report promptly any adverse effect, question, or relevant change in your condition.
- Attend complementary tests or in-person consultations when the physician indicates.
- Not share prescribed medications with third parties.
- Respect scheduled appointments and give timely notice if unable to attend.
- Have a private environment with good connectivity during the videoconsultation.
8. Costs and cancellation policy
Current service fees are shown at the time of booking and confirmed before payment. Payment is processed through secure gateways (Stripe) and we do not store your banking information.
Cancellation and rescheduling policy: you may cancel or reschedule without penalty up to 24 hours before your appointment. Cancellations with less than 24 hours' notice or no-shows may incur an administrative charge equivalent to the consultation fee. Medications already shipped are non-refundable except in case of manufacturer defect.
Signature of patient and attending physician
By continuing with the booking flow and signing in the corresponding step, the patient declares that they have read, understood, and fully accepted this informed consent. The attending physician will electronically sign this document at the start of the videoconsultation; both signatures are integrated into the clinical file.
- Patient: full name and digital signature (recorded at the moment of online signing, with timestamp).
- Attending physician: name, professional ID, and electronic signature recorded at the start of the videoconsultation.
